Health insurance for Virginia consultants outside Northern Virginia who need a PPO that travels

The short answer: Outside a handful of Northern Virginia localities, Virginia's 2026 marketplace sells HMO and EPO medical plans and no PPO, and full prices rose about 21.6%. In the tax credit range, a marketplace plan is usually the most affordable. Above it, healthy consultants can price an underwritten plan on a nationwide PPO network.

You consult from Richmond, Norfolk, Virginia Beach, Charlottesville or Roanoke, with clients in Charlotte one month and New Jersey the next. You invoice by project or retainer, pay your own premiums, and want a plan that works where the work happens.

For 2026, CMS's state marketplace plan files show a PPO medical plan on Virginia's Insurance Marketplace from a single insurer, sold only in a handful of Northern Virginia localities such as Arlington, Alexandria and parts of Fairfax County. Marketplace plans are sold by where you live, so anywhere else in Virginia your choices are HMOs and EPOs centered on your region.

Sort yourself with one question: will your household's income for the year fall inside the premium tax credit range? A single consultant gets no federal help for 2027 coverage, picked in open enrollment from November 1, above $63,840 of income ($62,600 for 2026), 400% of the poverty guideline, while families have a higher threshold.

If your billings leave you eligible for a credit

A slow quarter, a new practice or a year spent building the pipeline can put a consultant inside the credit range. In that case a plan from Virginia's Insurance Marketplace, which runs at marketplace.virginia.gov rather than HealthCare.gov, is usually the least costly coverage available to you, HMO or EPO included, because the credit is only usable on marketplace plans.

Project billing makes the estimate the hard part. A retainer you can count on is easy to project; a large engagement that closes in November is not. Advance credits are trued up on your tax return, and a stronger year than you projected can mean paying some or all of the credit back. The subsidy cliff guide shows how quickly help ends.

Away from home, a Virginia HMO or EPO steps in for genuine emergencies, because federal rules oblige marketplace plans to cover emergency care outside the network without prior approval and at in-network cost. Annual exams, refills and follow-up visits happen back in Virginia.

Next step: estimate around your pipeline. Sam Jaber can help you project the year from your retainers and open proposals and choose a Virginia plan that fits. Call 813-999-0101 if you would rather talk.

If you bill too much to get a credit

Above the credit range you pay the full premium, and Virginia's went up sharply. The State Corporation Commission's Bureau of Insurance reported an average increase of 21.6% before subsidies for 2026. That was with the state's reinsurance program still working underneath: the Commission has reported reductions of roughly 15% to 17% a year since the program began in 2023.

So a consultant past the line pays a higher full price, for an HMO or EPO that covers routine care near home and treats the rest of the country as emergency-only territory. For a lot of consultants above the line, that is more than the budget will justify. The next step depends on your health.

If you're healthy and work for clients across state lines

Healthy consultants often find the better answer off the marketplace. There are private plans that are medically underwritten and run on PPO networks that reach across the country. Since they are not sold through Virginia's Insurance Marketplace, the Northern Virginia boundary is irrelevant to them, and a doctor near a client in Philadelphia can be in network just as one in Henrico can.

What to expect:

What decides it is a direct comparison: the PPO plan's premium and network next to a full-price Virginia HMO or EPO for the same household.

Next step: compare a nationwide PPO with your Virginia price. Sam Jaber can quote one for your household and put it beside the marketplace, and the online form has no health questions. Call 813-999-0101 if you would rather talk.

If your health needs steady care

Ongoing treatment or a high-cost medication points back to the marketplace, even at full price. A plan there must enroll you and cannot price you by your health, while an underwritten plan could decline you or exclude the condition you need covered.

Your decision then is which Virginia HMO or EPO to choose. The companies serving your area depend on where you live, and each has its own doctor list and drug list, so check your specialists and prescriptions against every option before comparing premiums, and keep appointments to the weeks you are home.

Next step: choose the Virginia plan your doctors take. Sam Jaber can review the plans at your address against your specialists and medications. Call 813-999-0101 if you would rather talk.

If your coverage from a former employer is ending

Many consultants go solo after years at a firm. Larger employers offer federal COBRA, which continues the group plan for a limited time at up to 102% of its full cost. At smaller employers that COBRA does not reach, Virginia law, section 38.2-3541 of the Code of Virginia, lets you continue the group coverage for 12 months, provided you were covered for at least three months before it ended.

When either one runs out, that loss of coverage opens a special enrollment window on Virginia's Insurance Marketplace, and you can usually choose a plan before the end date so nothing lapses. If you are healthy and above the credit range, price an underwritten nationwide PPO plan in the same comparison.

Next step: have the next plan ready before the old one stops. Sam Jaber can set continuation, a marketplace plan and a nationwide PPO side by side against your end date. Call 813-999-0101 if you would rather talk.

If you live in Northern Virginia instead

If home is in one of the localities where the marketplace PPO is sold, you have one more option to weigh. It is still a marketplace plan, so it must accept you regardless of health and it is the plan a tax credit can be applied to. Before you assume it travels with you, check how far its network reaches outside the region, then price it next to an underwritten nationwide PPO if you are healthy and above the credit range.

Next step: line up the three options. Sam Jaber can set the Northern Virginia marketplace PPO, an HMO or EPO, and an underwritten nationwide plan side by side. Call 813-999-0101 if you would rather talk.

Questions Virginia consultants ask

Can I buy the Northern Virginia marketplace PPO if I live in Richmond?

No. Marketplace plans are sold by where you live, and the one PPO on Virginia's marketplace for 2026 is offered only in a handful of Northern Virginia localities. Driving to clients in Arlington every week does not change which plans you can buy.

Is private health insurance better than the marketplace for a consultant?

It depends on two things: your income and your health. With income in the credit range, the marketplace is usually less expensive. Above that range, a healthy consultant may do better with an underwritten plan on a nationwide PPO network, while someone with an ongoing condition is usually better off staying on the marketplace.

How much did Virginia marketplace premiums go up for 2026?

About 21.6% on average before subsidies, according to the State Corporation Commission's Bureau of Insurance. That increase came even with a state reinsurance program in place since 2023.

Do I make too much for Obamacare in Virginia?

For 2027 coverage, chosen in open enrollment from November 1, a single person with income above $63,840 gets no federal premium tax credit, since that is 400% of the poverty guideline; for 2026 the line is $62,600. The limit is higher for larger households. Virginia's new state help for 2027 is aimed at lower incomes, between 138% and 250% of the poverty level.
